Transition Planning Services



Transition Planning is offered in Nova Scotia and New Brunswick to Grade 9-12 students who are Deaf and Hard of Hearing.


This service is offered by the Transition Planning Facilitator (TPF), who collaborates with students and their families, school staff and community agencies. The TPF will participate in school initiated transition planning, or moderate (take the lead) if appropriate.


The purpose of Transition Planning is to assist the student in long range planning and developing strategies to achieve his or her goals. This includes awareness raising and the development of advocacy skills, either in individual sessions, group activities or career related short-term programs.


Individual Transition Plans usually include documented time lines for steps to raise awareness of self and the world of education and employment. This may be by means of career exploration activities, "Work Experience", "Job Shadow", visits to community agencies, post-secondary training and education institutions, etc.
